HOLD is a scheme which provides shared ownership for those with long term disabilities. To qualify you need to:

Require a new home better suited to your needs

Have an agreed support package in place or ready to start

Be capable of living independently (with support)

Be able to afford the admin costs of buying a house (approx £3,000)

Have sufficent income or benefits to afford home ownership
